Medication Management

A health care professional will come to your home for a wellness check and sort your pills into easy open containers providing for individual doses. They will handle all prescription refill needs with your doctor. In addition, optional daily medication reminders can be programmed through the Lifeline CarePartner telephone service.

The MOST Important Reasons to Use the Medication Management

INDEPENDENCE
Enables you to maintain your INDEPENDENCE. A professional sets up your medications, following your Doctor's instructions. This improves compliance and reduces the risk of medication errors. YOU CAN STAY INDEPENDENT AND IN YOUR HOME LONGER.

HEALTH
Did you know that taking medications correctly could be the MOST IMPORTANT LIFESTYLE CHOICE YOU CAN MAKE? Almost everyone makes a medication mistake.. Patients who take medication correctly are more likely to stay well, make fewer clinic visits and require fewer hospitalizations.

FINANCES
Health care costs are frightening everyone. This is an easy way to keep the cost of your health care down. You may save by reducing emergency room visits due to medication errors, fewer Doctor visits and possibly fewer prescriptions. Moving to a facility where they manage your medication would be a much greater monthly cost.

CONVENIENCE
This service is CONVENIENT. Your supply of medications, packaged by dose is setup for you right in your own home. Any changes needed and all refills are handled for you. You needn't worry about driving to the pharmacy, or standing in line while the person next to you hears YOUR medical history. A health care professional is just a phone call away to answer any questions.

Patient Assistance Programs

Many of the pharmaceutical manufacturers offer Patient Assistance Programs (PAPs) to those with no insurance and/or low income. These programs offer great assistance to many on expensive medications. The assistance ranges from discounted pricing for medication to even some being offered for free. Each program is slightly different and each has certain criteria that must be met. Always let your doctor know if you can't afford the medications they prescribe. There may be generic or alternative medications available, they can provide you with free samples, or assist you with signing up for the PAPs.
