The
following, listed in alphabetical order, is a brief description
of all the services.
Assessment
(Initial) and Regular Visit by a Professional: An
initial wellness assessment addressing physical, emotional
and social concerns can be performed by a licensed professional,
which serves to identify the needs of the client, as well
as acting as a baseline for future assessments provided
on a monthly or quarterly basis. This can be done by a
Care Manager or RN, depending on the level of need by
the client. The findings of these visits can be communicated
to the caregiver via a password driven login on the Easy
Living Website, e-mail or a phone call.
Care
Management: Our Geriatric Care Managers are experienced
professionals who assist older adults and their families
in coping with the challenges of aging and in the planning
of long term care. Care Management may include:
- Act
as a liaison to long distance caregivers by regularly
communicating their family member's progress or alert
them of any problems.
- Care
planning assessments to identify the problems and needs
that require services.
- Screen,
arrange and monitor all home health and other services.
- Provide
consumer education and advocacy.
- Offer
counseling and support.
- Provide
crisis intervention.
The
Care Manager initiates a personalized care plan, specifically
designed to meet the client's wants and needs. They help
to reduce the time, stress and additional costs often involved
in caring for an older adult as they age.
Cleaning
(House):
Qualified and carefully screened professional cleaning
companies provide the cleaning component of our services.
House Cleaning is based on a 1 or 2 bedroom home up to
approximately 1200 sq. ft cleaned on a monthly cycle.
If your home is larger, needs special attention, or if
you would like to increase the frequency, there may be
an additional charge monthly. The regular house cleaning
includes:

sinks, & accessories

Coordination
and Scheduling of Services: The detail oriented scheduling
of providers and coordinating with the client or family
is included. Our experienced coordinators coupled with
the eZ Link software offers us the ability to handle multiple
tasks in an efficient and expeditious manner.
Direct
access to Personal Support & Services: Our state
of the art emergency response systems provide our clients,
as well as their caregivers, with peace of mind, knowing
that someone is available when a need arises, twenty four
hours a day. As a client advocate, Easy Living will assist
the client to set up services to meet their individual
needs, along with access to any services that can be arranged
at little or no charge through Social agencies or those
that are covered under their medical insurance. The eZ
Link software on our Website gives the client or caregivers
direct access to services, schedules, and Provider comments
if desired with Care Management service and the clients
permission.
Doctor
House Calls: Getting to the Doctor becomes more difficult
as one gets older, especially when one can no longer drive.
Many seniors just don't go as often as they should, creating
a detrimental effect on one's health. We can arrange for
a Doctor or Nurse Practitioner to visit the senior at
home. Even a number of basic tests can now be handled
right in one's own home.
Home
Health Aides: All of our Health Care companies are
licensed, insured, and are put through the most stringent
background checks. In addition, a personal care plan is
provided by a registered nurse, which is carefully followed
by the aide.

Visit:
A Home Health Aide provides assistance with bathing,
light meal preparation, and dressing and is no longer
than a 1-hour visit.
Hourly:
Four or more hours per day expand the health assistance
and support offered and is charged at an hourly
rate. The additional time spent with your loved
one provides not only the hands-on care, but adds
the important stimulation through activities, along
with supervision by a RN. In addition, the Aide
will be able to prepare additional meals, do some
lighthouse work, shopping, and assist client as
requested.

Homemakers:
All of our homemakers work for companies that are licensed,
insured, and are put through stringent background checks.
In addition, a personal care plan is provided at the beginning
of care, which is carefully followed by the staff. A manager
will then supervise the aide and the clients care, as
required by law, making changes whenever appropriate.
The homemaker will be able to take care of normal household
chores, such as to preparing meals, lighthouse work, laundry,
shopping, errands, and medication reminders, but cannot
assist the client with any physical or personal care.
With a minimum of three or more hours per day, a homemaker
is charged at an hourly rate. The time spent with your
loved one provides not only the household help needed,
but adds the important stimulation through companionship
and activities, along with supervision by a professional.
Meal
Program: Our meals consist of several options

Basic:
The first alternative offered is an array of nutritious
frozen meals, of which a minimum of 7 per week
are delivered right to the clients home.
These meals are flash frozen immediately after
they are prepared. Just heat them in the oven
or microwave and you are ready to enjoy one of
the more than 30 selections to choose from.
Personal
Chef: A "Personal Chef" prepares
from 20 up to 30 meals right in your own kitchen.
The "personal chef" conducts an interview
to mutually determine your preferences for the
menu, buys only the freshest ingredients, and
then prepares the gourmet meals on your scheduled
cooking day. The chef packages and then refrigerates
or freezes each item, leaving thawing and re-heating
instructions, so your meals will taste like they
were just prepared. Our chef will then clean your
kitchen and leave your home smelling wonderful.

Medical
Alert: A personal emergency response system (PERS)
or sometimes called a medical alarm system. The system
includes a waterproof pendant to wear on your wrist
or around your neck and a base unit attached to a telephone
jack and electrical outlet. With just a push of the
button, the system puts you quickly in contact with
a qualified professional 24 hours, 7 days a week for
an emergency or to just have someone to talk to from
anywhere in your home. Being able to communicate and
stay in contact with others is often difficult for individuals
living alone, but so critical by receiving an immediate
response when an emergency occurs. As an independent
company serving the needs of seniors, we have researched
companies, tested the equipment, and evaluated the various
systems on the market. We work with the most technologically
advanced, quality systems and companies in this field.

Medical
Equipment: There are many aids to assist seniors
with certain tasks as they become more difficult, with
health monitoring, as well as your safety and comfort.
They include ambulatory aids (canes and walkers, wheelchairs,
electric lift chairs, scooters and electric wheelchairs),
pillows and cushions, bathroom safety items (bath seats,
grab bars, etc.) home diagnostics (blood pressure monitors,
etc.), braces and supports, and assistive devices (reachers,
dressing and eating aids, etc.). Some of these may be
provided by Medicare with your doctor's prescription.
Medication
Management: SafeDose organizes your medications
at the pharmacy. Your pills are inserted into easy open
individual packets organized in sequence by day, date,
dose and time to improve convenience and reduce the risk
of medication errors. Patients who take medication correctly
are more likely to stay well, make fewer clinic visits

Prescription
Delivery Service: As a member of the Easy Living Program,
you are automatically eligible to take part in our "Free
Delivery" of all your prescriptions and over the
counter medications. SafeDose Pharmacy fills the
necessary prescriptions that you require with accuracy
into dose packets and conveniently deliver them to your
home on a monthly cycle. All participants receive only
the highest quality FDA approved prescription drugs, which
are always checked for compatibility, billed to your credit
card, to your home or if covered, to your insurance carrier.

Recreational Stimulation: A Homemaker prevents
an individual from feeling isolated and often offers motivation
to take part in recreational activities. They are there
to read, play cards or take your family member on a day
trip to the mall, movies, beach or museum. They can also
do daily chores together such as shopping, errands, or
light cleaning, but where no physical assistance is needed.
We have found that this type of stimulation can help to
increase self-esteem and improve the quality of one's
life. If hands-on care were needed, a Home Health aide
would be required.
Reminder,
Check-up, or Wakeup Calls: The daily, weekly or monthly
check-up calls to the client are designed to serve many
purposes. The calls open up a line of communication with
the client. The client will feel less isolated knowing
that they will have someone to talk to on a regular basis.
They help to develop a trusting relationship with the
client, which then helps with the acceptance of the other
needed services. These calls may be set up through the
Personal Emergency Systems and used as a reminder service
for taking medications, scheduled appointments, or a wake
up service.
Transportation:
During our research, transportation was identified as
being one of the most sought after services. Whether you
would like to go shopping, to your doctor's office or
to a special event, our courteous drivers will be there
for you. A companion, homemaker, or HHA can assist with
transportation.
Quality
Check: The quality check calls to the client are proactive
and designed to serve several purposes. It helps to keep
all our services at a level of excellence, helps with
the acceptance of the services, and more importantly,
makes certain that the client is fully satisfied. Any
Providers not meeting our stringent Quality requirements
are dropped from our network.
Website
Access to Client Services: Family members or caregivers
who are providing assistance to an elderly adult in a
long distance care-giving situation often do not truly
know how their loved one is doing. They also do not always
know what services or options are available to their family
member. With Website access the caregiver can view scheduled
services, such as, reports on the monthly assessment by
a professional or provider scheduling of regular visits
with the client. Through Care management notes it serves
to report the progress or identify the lapses in the client's
wellbeing and as a result, may accelerate the caregiver's
awareness of necessary care a loved one might require.
